Effective Hemostasis Ensures Isolation During Retraction
When cementing a crown into place, ensuring that the cement is not contaminated by blood is an imperative, as this may lead to recurrent decay. Therefore, effective hemostasis and retraction are essential to the performance of this procedure-and many others in daily practice.
"Anywhere that we have tissue that's irritated, we need it to be dry and free from contaminants," says G. Franklin Shull, DMD, of Palmetto Dental Associates in South Carolina. "For example, crown deliveries, someone who is not flossing properly, a temporary that's not perfect, a Class II below the tissue level-anywhere that you're trying to have a better field of isolation from blood. Dryz Blu® has tons of uses in the general practice in addition to impressioning."
In a recent interview with Product Talk's Michael DiTolla, DDS, Shull explains the advantages of Parkell Dental's Dryz Blu, a gingival retraction paste that includes a hemostatic agent. With its 25% aluminum chloride content (most other products in this category have only 15%), it provides fast, effective hemostasis, and its stiff consistency facilitates greater displacement of sulcular tissue. Dryz Blu is designed for tissue management when seating restorations, placing rubber dam clamps (to control bleeding), bleaching teeth (to manage moisture), and restoring subgingival cavities.
Another feature of the product is its small, 1-mm diameter tip, which assists in getting the material into the sulcus. Clinicians can hold it at a 90-degree angle against the tooth, go tip-down apically at 45 degrees, and then pull down on the gingiva.
"You can play around with different techniques," Shull says. "I focus on the material going subgingival, so slightly retracting the tissue with the rounded end makes a lot of sense. Other products I've used, keeping them in place has always been the challenge. This material stays where you put it."
It's also critical that the sulcus be free from any contamination when taking impressions. "Impressioning is something that we all struggle with," says Shull. "We need to keep the field isolated, and we always worry about contaminants like blood if we nick the tissue or have a subgingival margin. Having a product that we can easily dispense and have really good hemostasis is really important."
For added confidence, Parkell is launching an impression cap that will be shipped as a sample with orders of Dryz Blu. Clinicians place the retraction paste into the sulcus, put the compression cap on top, and then have the patient bite down. In addition to the aluminum chloride activity of Dryz Blu, the pressure from the cap pushes the blood from the capillary beds, improving performance.
"It's easy to dispense," Shull explains. "Then the compression cap helps the patient press the material into the sulcus. Wait for 2 to 3 minutes, take it out, rinse it off, and you're ready to go. During that time, I can see a hygiene patient."
Most clinicians are accustomed to a two-cord technique, and Shull believes that's still a good option. "I'll often use a cord in the base of the sulcus, but then I'll go on top of that with Dryz Blu to replace the second cord and put the compression cap on. That technique works really well for me," he says.
With maxillary premolars or lower anterior teeth, there's thin, friable gingiva present, and the clinician might do damage if two cords are placed in the sulcus. In cases like this, Dryz Blu and the compression cap provide a valuable alternative.
