James H. Peyton, DDS, has a passion for performing cosmetic dentistry, especially placing composite restorations. “Doing an excellent job and creating a pleasant experience for my patients is essential,” he says. “And part of that involves maintaining good communication with my patients so that they know what needs to be done and how we are going to do it.”
Peyton has spent years building his Bakersfield, California, practice into the successful cosmetic dentistry practice that it is today. “I treated my patients well, and they referred their family members and coworkers to our office,” he says. “After 12 years, we moved into a nice Dutch colonial building with more space on a busy street in the downtown area. We put together a website that would better display our cosmetic cases, and I also joined the American Academy of Cosmetic Dentistry and became an accredited fellow several years after.”

Your wipes cost too much
One material that has helped Peyton deliver excellent esthetic results in his cosmetic cases is OMNICHROMA® one-shade universal composite from Tokuyama Dental. He was given the opportunity to preview and test it before it launched and was highly impressed. “I was amazed to see how OMNICHROMA blended into the different shades,” he says. “It was like magic.”
OMNICHROMA is a universal composite that is able to esthetically match every one of the 16 VITA classical shades, from A1 to D4, with a single shade. This is accomplished through Tokuyama Dental’s Smart Chromatic Technology, which leverages uniformly sized supra-nano spherical fillers (ie, 260 nm spherical SiO2-ZrO2) that are the exact size and shape needed to generate the red-to-yellow color of natural tooth structure. As light passes through the cured composite, the fillers reflect the color of the surrounding tooth structure, producing a perfect shade match. In, addition, OMNICHROMA’s excellent physical properties provide exceptional handling, high compressive strength and wear resistance, and superb polishability. “OMNICHROMA does an excellent job of taking on the shade of the natural tooth structure,” Peyton says. “It’s handling properties and ability to be polished to a high gloss are similar to that of Tokuyama Dental’s other composite lines, Estelite Sigma Quick® and Estelite Omega®; however, with OMICHROMA, no shade-taking procedures are required, and the results are achieved with just a single syringe of composite.”
Not having to perform a shade-taking procedure will eliminate the opportunity for mistakes that may result in a poor shade match and reduce the overall chair time required for treatments, which saves time and improves productivity. Furthermore, a single-shade material reduces composite inventory and waste from infrequently used shades, saving practices money. “As a cosmetic dentist, I understand that there’s a place for layered composite restorations,” Peyton says. “But being able to deliver a great result with a single shade of composite saves a lot of time as well as money in material costs.”
The OMNICHROMA family includes a lower viscosity formulation, OMNICHROMA Flow, which is also available in a bulk-fill option, OMNICHROMA Flow BULK. “I use the OMNICHROMA Flow on small chips and in the deep areas of larger restorations,” Peyton says. To prevent shade-matching interference from stained areas or from the shadows of the mouth in situations that lack sufficient surrounding tooth structure, such as class III and class IV restorations, OMNICHROMA BLOCKER should be used in conjunction with OMNICHROMA. “OMNICHROMA works great alone in most clinical situations; however, if the restoration involves deep discoloration or a fractured incisal edge, you will probably need to use OMNICHROMA BLOCKER,” Peyton says. “I like to use the BLOCKER for the lingual aspect of large class IV fractures and in deep dentin areas. Then, I use OMNICHROMA to fill in the rest of the restoration.”
Peyton was gracious enough to share a recent case that highlights one of the treatments for which he routinely uses OMNICHROMA in his cosmetic dentistry: black triangle closure in the esthetic zone. “This female patient was very concerned about the dark triangles between teeth Nos. 7 through 10 and wanted to get rid of them,” Peyton says. “During the first appointment, teeth Nos. 8 and 9 were treated. There was no need for an anesthetic. To conservatively prepare the teeth, the stains and debris on the surface enamel were removed with coarse contouring discs and interproximal finishing strips. This created an excellent surface to bond the restorations. Teeth Nos. 8 and 9 were then acid etched with 35% phosphoric acid, after which a bonding agent was applied and light cured. Next, OMNICHROMA was applied to the mesial-facial and distal-facial surfaces of tooth No. 9 and contoured with a thin interproximal carver. OMNICHROMA has nice handling properties, so sculpting and contouring the material was very easy. A No. 3 brush was also used to smooth the composite surface. After tooth No. 9 was light cured, tooth No. 8 was similarly restored. During a subsequent office visit, teeth Nos. 7 and 10 were restored in the same fashion so that all of the dark triangles in the patient’s esthetic zone were no longer visible. The patient stated that she felt the color blended in nicely. She was very grateful to have a nice smile and no more dark triangles.”
