Pearl has launched Second Opinion 3D, making its FDA-cleared artificial intelligence technology for CBCT imaging commercially available to general dental practices in the United States and Canada. According to the company, Second Opinion 3D is designed for chairside use and is integrated into the same Second Opinion interface used for 2D dental imaging.
The commercial release adds the ability to request reports from licensed oral and maxillofacial radiologists within the platform and automatically convert CBCT findings into documentation for insurance claims.
The launch follows FDA 510(k) clearance received by Pearl in 2025 for its 3D radiologic image analysis technology. According to the company, the AI automatically identifies anatomical structures in CBCT scans, including the mandible, maxillary complex, nasal space, airway, inferior alveolar nerve canal, and individual teeth.

Second Opinion 3D automatically segments and color-codes anatomical structures for visualization, according to the company. Its interactive viewer allows clinicians to remove or highlight specific structures and includes 3D rendering, zoom and rotation, opacity controls, and length measurement.

According to Pearl, the platform’s radiology-report function allows clinicians to request a specialist review without exporting a scan to a separate portal. The software can also convert CBCT findings into insurance claim documentation and create 2D images from CBCT scans for claims.
Second Opinion 3D also includes AI-powered pathology detection for findings including temporomandibular disorders (TMD), carotid artery calcifications, impactions, periapical radiolucency, and sinus radiopacities. According to the company, these capabilities are currently available in Canada and are pending regulatory approval in additional markets.
