Dentira has released its inaugural Dental Spend Index (DSI), a semiannual report examining dental supply purchasing patterns, formulary compliance, and supplier strategies among dental service organizations (DSOs). According to the company, the Fall 2026 report draws on procurement data from 267 DSOs representing 15,000 locations across the United States.
The Index uses purchasing, formulary, budget, and invoice activity recorded through Dentira's procurement platform to provide benchmarks by DSO size and supply category. The company states that findings are reported at the segment and category levels to maintain confidentiality between buyers and suppliers.
Among the report's findings, DSOs with 101 or more locations spent an average of 4.1% of collections on supplies, compared with 7.4% among organizations with 10 or fewer locations. Dentira identifies this 3.3-percentage-point difference as a potential area of focus for procurement teams evaluating supply costs.
The report also found that prices increased across all 10 of the supply categories examined between 2021 and 2025. Labels and signs recorded the highest reported increase at 20.1%, followed by disposables at 15.8%.
Formulary compliance was another area examined in the Index. According to Dentira, approximately half of the products ordered by DSOs in its dataset were outside their formularies. Enterprise DSOs recorded an off-formulary ordering rate of 45%.
The report also examined DSO growth and acquisition activity. Dentira reported growth rates of 32% for small and midsize business DSOs, 26% for mid-market organizations, and 16% for strategic DSOs. Enterprise DSOs increased their reported growth rate from 18% to 26%. Among DSOs on the Dentira platform, the South Atlantic region accounted for nearly one-third of acquisitions over three years, while Florida, Texas, and California recorded the highest acquisition activity among individual states.
In its analysis of supply categories, Dentira found that implants and bone regeneration materials ranked among the four highest spending categories across every DSO segment.
The company also reported that customers using its procurement platform reduced supply spending by an average of 1.1 percentage points of collections during their first year, equivalent to a reduction from 8% to 6.9%.
Every order, invoice, and formulary decision on our platform adds to the most complete picture of dental spend the industry has had, said Vik Gupta, co-founder and CEO of Dentira. We built the Dentira Dental Spend Index to help procurement leaders benchmark against their peers and plan with evidence.
As a dentist and a DSO founder, I know every supply dollar competes with a new hire, a new operatory, or a new service line, said Michael Benarroch, co-founder and chief strategy officer of Dentira. The leaders I talk with want control of spend and confident clinicians. This Index shows them where to start.
The Index also includes the Dentira Dental Spend Maturity Model and a 90-day plan for procurement teams. Dentira expects the Spring 2027 edition to add a category-level formulary analysis.
Founded in 2020 and headquartered in San Francisco, Dentira provides procurement technology for dental organizations and independent clinics. According to the company, its platform serves more than 250 DSOs and 17,000 clinics across seven countries, connecting users with more than 800 suppliers and 3.1 million stock-keeping units. Dentira is backed by Vista Equity Partners.
