Simplifying Shade Selection for Esthetic Restorations

I am a very picky dentist. Patient care is incredibly important to me, and I am fortunate to have a loyal patient base because they recognize how much I genuinely care about their dental health and outcomes. While I maintain a general dentistry practice, I perform a significant number of full-mouth reconstruction cases, and cosmetic dentistry remains my favorite part of practicing dentistry.

My professional journey began at the University of Pennsylvania School of Dental Medicine. After graduation, I spent many years studying at the Dawson Academy and serving in leadership positions within the American Academy of Cosmetic Dentistry (AACD).

I have also been fortunate to become involved with Catapult Education, which has provided opportunities to lecture extensively while learning about innovative dental products and companies. I was first introduced to Tokuyama Dental’s Estelite Omega at an AACD meeting and eventually began teaching seminars and hands-on programs featuring the material because it consistently produced some of the finest anterior restorations I had ever placed.

Several years later, I was introduced to a product that I consider one of the most fascinating and successful materials I have ever used: OMNICHROMA by Tokuyama. I had the opportunity to work with OMNICHROMA for a full year before its release in the United States, and I was immediately impressed.

OMNICHROMA’s ability to blend seamlessly with virtually every tooth shade eliminates the need for maintaining multiple composite shades. It remains my primary composite for a wide variety of esthetic restorations. Over time, I also incorporated OMNICHROMA Flow and OMNICHROMA Flow BULK into my restorative workflow.

Initially, I was somewhat skeptical about using OMNICHROMA in anterior esthetic cases. Like many dentists, I questioned whether it could truly match the esthetics of traditional anterior composites. Fortunately, my concerns proved unfounded.

I routinely use OMNICHROMA for diastema closures, Class V restorations, Class IV restorations, and the treatment of white spot lesions and other anterior discolorations. The shade-matching capability is remarkable regardless of the underlying tooth shade. Even more impressive, OMNICHROMA blends beautifully across multiple shades within the same tooth and integrates seamlessly with both enamel and porcelain restorations.

For me, the most important characteristic of a composite is its handling. OMNICHROMA handles exceptionally well. It adheres to the tooth rather than the instrument, making placement far more predictable. The material is less sensitive to ambient operatory light than many composites and provides ample working time compared to many other products. Finishing and polishing are straightforward, and the final luster is outstanding.

For my patients who whiten their teeth, I have found that OMNICHROMA restorations continue to blend naturally as the surrounding tooth structure lightens. This can reduce the need for replacing restorations after whitening procedures.

OMNICHROMA BLOCKER and OMNICHROMA BLOCKER Flow are equally intriguing materials designed to be placed beneath OMNICHROMA—or other composite systems—when additional masking is required. I truly enjoy working with these materials because they function as opaquers without the drawbacks traditionally associated with highly opaque products.

Most dentists have experienced the frustration of using conventional white opaquers. While they effectively mask dark stains and discoloration, they often create a new problem: the opaque white material shines through the restoration, resulting in an unnatural appearance.

OMNICHROMA BLOCKER is different. It effectively masks dark amalgam staining, metal show-through, and underlying discoloration without creating an artificial white appearance. Because it is not excessively opaque, it allows the overlying OMNICHROMA to blend naturally with the surrounding tooth structure rather than imposing its own shade. In most anterior cases, I use approximately 0.5 to 1.0 mm of OMNICHROMA BLOCKER beneath 2 mm to 3 mm of OMNICHROMA.

One of my favorite applications involves implant screw-access channels and metal post access holes. I developed a technique using OMNICHROMA BLOCKER to mask the metal and dark underlying structures, followed by approximately 3 mm of OMNICHROMA sculptable placed over plumber’s tape. The results have been remarkably predictable, effectively eliminating the gray discoloration that has challenged these restorations for years.

If you have not yet worked with the OMNICHROMA family of products, it may be worth evaluating in your practice. In my experience, Tokuyama has consistently provided excellent customer service and support, making the learning process straightforward.

For dentists new to the material, I recommend beginning with posterior restorations such as Class I or Class II cases before progressing to more demanding anterior esthetic applications. You may be surprised by how easily a single composite shade can satisfy the vast majority of your restorative needs. You will likely also appreciate the economic advantages of maintaining fewer composite shades and eliminating the waste associated with expired inventory.

As stated earlier, I was initially resistant to using OMNICHROMA on anterior esthetic cases other than Class V restorations because I like to use a dentin shade and a more translucent enamel shade. As I became more comfortable using the material, I have continued to be very happy with the results I have achieved using OMNICHROMA alone or with OMNICHROMA BLOCKER.

A clinical case shows treatment of a lower diastema in a young woman. I used OMNICHROMA sculptable and did not need to use OMNICHROMA BLOCKER. As you can see in the clinical photographs, the material blended very well with the varying shades present throughout the teeth.

Another case demonstrates removal of decay and repair of an upper crown. I have also found that OMNICHROMA matches porcelain as well as enamel. In this case, OMNICHROMA used with OMNICHROMA BLOCKER created a virtually ideal match.

In my experience, OMNICHROMA Flow and OMNICHROMA Flow BULK are excellent options for smaller restorations and provide additional versatility within an already impressive restorative system.
